In light of concerns surrounding coronavirus and in response to the latest recommendation from the White House, we have suspended all non-emergency care for at least the next two weeks – through the end of March. If you have an appointment scheduled within that time period, we will contact you as soon as we know when to reschedule. This will help us to manage the overall appointment calendar in order to best address everyone’s needs. We do not want to schedule appointments until we know the actual date that our office will reopen. If you have a true dental emergency during this time, such as pain or injury, we will be available for emergency appointments. Please call our office (there is an emergency contact number for Dr. Weeden) so we may determine the best course of action for your specific situation. New Patient Forms

The following forms should be completed prior to the first visit. Please have the completed forms ready for the New Patient Coordinator at your first appointment:

  • Notice of Privacy Practices: This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
  • New Patient Questionnaire: The questionnaire includes contact and insurance information as well as your medical and dental history. You may fill out this form online and submit it via secure email, or print it off, fill it out, and bring it with you to your first appointment.
